How to Register as a patient at The Access Place

The people we support:

People experiencing homelessness, who have additional needs, aged 16 and over who:

- Are sleeping rough
- Are temporary residents of hostel accommodation provided by the council
- Are in unsupported temporary accommodation
- Are sofa surfing
- Have a history of homelessness (as defined above and are at high risk of becoming homeless again because of ongoing complex health, housing or social care needs.

Every effort is made to ensure those under 18 are re-directed to the right service.

To register with our practice, please come into reception to get a form, or complete the appropriate registration form at https://www.edinburghaccesspractice.scot.nhs.uk/page1.aspx?p=4&t=1.

You can only be on the ‘list’ of one GP Practice at a time. For this reason, if you want to be considered for the TAP ‘list’, you will require to tell us who your previous GP was, or where the surgery was which last provided you with medical care.

We can usually find that information for you if you are unsure of the name and address of your last GP. We will make contact with your previous GP to check what treatment you have been receiving.

We cannot provide treatment for you without contacting your previous GP to access your notes.

Once we have your forms:

- We will request your medical records from your previous GP.
- It will take at least 2 working days before you are able to book an appointment with a clinician.
- We will book you a New Patient Health Assessment with one of our Practice Nurses which will take approximately 1 hour.
